Preterm Delivery

Preterm delivery occurs at least two weeks before the expected due date and is a major factor in perinatal morbidity and mortality worldwide. In developed countries, preventing and managing preterm birth is a key focus of modern prenatal care.

One of the most effective screening tools for assessing the risk of preterm delivery is ultrasound measurement of cervical length. This examination is an essential part of the fetal anatomy scan during the second and third trimesters, as early detection of risk factors for preterm birth plays a crucial role in improving pregnancy outcomes.

Cervical Length and Preterm Delivery Risk

  • A longer cervix is associated with a lower risk of preterm delivery.
  • A shortened cervix increases the risk progressively.
  • A cervical length of less than 2.5 cm is considered high risk and requires further assessment.

 

When and How is Cervical Length Measured?

  • The ultrasound assessment of cervical length is typically performed around 20 weeks of gestation as part of the fetal anatomy scan.
  • It can be conducted transabdominally and/or transvaginally.
  • The transvaginal approach provides significantly greater accuracy.
  • In most cases, an initial transabdominal measurement is performed, followed by a transvaginal scan if necessary.
  • Before 16 weeks of gestation, cervical length measurement does not provide clinically relevant information.
