Neonatology


The Neonatology Department offers state-of-the-art conditions for the treatment of healthy newborns and semi-intensive care, which includes the use of state-of-the-art medical equipment as well as educated staff. The mother / child system (rooming in) is implemented in the department.

In the days after birth, newborns are given ultrasound and clinical screening of the hips, screening for hypothyroidism, echocardiographic and ultrasound examination of the CNS, treatment of postpartum jaundice, administration of BCG and Hepatitis B vaccine. Within the department there is modern laboratory diagnostics as well as the possibility for application of digital X-ray diagnostics.

The department has a special space for isolation of newborns with postpartum infections and their appropriate professional and timely treatment with adequate therapy, as well as 24-hour monitoring. The department is equipped with state-of-the-art transport incubators for newborns as well as a highly sophisticated, own ambulance transport vehicle available 24 hours a day. The semi-intensive care has 3 incubators with accompanying equipment for intensive care (infusiomas, perfusors, monitors, photo lamps, warmers, herb, ultrasound device).

Services offered by the neonatology department:

  • Prenatal education for parents and care of a newborn
  • Education for mothers for successful breastfeeding according to the system “ten steps to successful breastfeeding” by a team of highly educated nurses.
  • Resuscitation, stabilization and treatment of newborns
  • TSH screening
  • Orthopedic and cardiological screening
  • Transcutaneous (bloodless) diagnosis of bilirubin, as well as treatment of hyperbilirubinemia with phototherapy

There is a clinic in the neonatology department where the newborns are permanently monitored in their growth and development. The neonatology team is composed of three neonatologists and highly educated medical staff.

26 November 2024Live laparoscopic gynaecological surgery from Hospital Plodnost was performed for the first time in Macedonia.

Recently, a symposium on microinvasive surgery in gynaecology and obstetrics in Macedonia was held in Skopje at the Holiday Inn Hotel, attended by international speakers.

Dr. Tashe Trpchevski, a prominent specialist, delivered a lecture accompanied by slides and video presentations, sharing the experiences of Hospital Plodnost in applying micro-invasive surgery—laparoscopy and hysteroscopy—to treat gynaecological conditions.

Simultaneously, Dr. Hristijan Trpchevski performed a live demonstration of a surgical procedure directly from the operating room in the hospital in Bitola. The surgery involved a patient with a gynaecological condition and included the removal of the uterus and lymph nodes.

“I am thrilled that this is the first live presentation of such a complex surgery in Macedonia, broadcast directly from the operating room. The procedure involved removing lymph nodes near blood vessels, a challenging and intricate operation. First, I must emphasize that very few gynaecologists in Macedonia perform such procedures laparoscopically. Second, this is the first time this complex and delicate surgery has been broadcast live to a large audience of experts in this field. Having the courage—and I would even say audacity—to present this technology to competent professionals who understand the intricacies of this field demonstrates mastery of the technique, as well as the skill and knowledge required for its execution.

We aim to share our knowledge and experience with other colleagues interested in this field. At the same time, we want to assure and inform our peers that Hospital Plodnost has become a reference center for what is known as advanced laparoscopic surgery. 30 August 2022This method is already adopted in our hospital and is only used here in Macedonia. In patients with cancer of the genital organs, in the specific case cancer of the endometrium (the lining of the uterus), latest trend is removal of the uterus, fallopian tubes and ovaries and lymph glands in the pelvis (pelvic) and outside the pelvis (para-aortic and paracaval), by laparoscopic way. This way, the lymph glands are mapped and the stage of the disease is determined.
“We apply this method in patients with low and high risk degree. In this particular case, the patient was 64 year-old, had high blood pressure and overweight. This procedure, in addition to the skill of the surgeon to remove the lymph nodes, also requires expensive laparoscopic equipment with a 4K camera that uses IR rays. We have such equipment. The patient only has three small cuts on her stomach, half a centimeter each, and she left the hospital on the third day. The same evening on the day of the operation, she was mobilized. This is the latest trend in the world.” says Dr. Hristijan Trpcevski, young but very experienced and skilled laparoscopist.

11 August 2022Two months ago, a laparoscopic lifting of the uterus was performed in a young 30-year-old woman with subtotal prolapse of the uterus (falling out of the vagina).
We left time to see how the patient will recover and how she’ll feel. She is fine and overjoyed that her uterus has been preserved. Namely, in these women, removal of the uterus through the vaginal route is done. We used a method which preserved the uterus. We performed a laparoscopic lift on the pectineal ligament of the pelvic bone using a mesh. Given that we had experience with lifting the vagina (vault prolapse) in 3 previous patients in a similar way, we performed the operation in just one hour. The patient left the hospital the next day.
Another alternative way of lifting prolapsed uterus is sacrocolpopexy, associated with more complications, the appearance of stress incontinence, plus it requires longer hospitalization and longer analgesia.
“This is a new method for which experiences are still being gathered in the world. 26 July 2022Only in Hospital Plodnost in Macedonia – third operation in a row of this kind!!!
In an older woman, with a previously removed uterus due to weakness of muscles and ligaments of the pelvic bottom, the vagina drops out of the body. An operative correction was also performed via the vaginal route elsewhere, without success.
“We applied a new laparoscopic vaginal lift technique to the pectineal ligament on the inside of the pubic bone in the abdomen. This is a new technique and the initial results show that it is much better than the so-called sacropexy, when the prolapsed vagina is lifted by the posterior pelvic bone (sacrocolpopexy). After such procedure, a high percentage of women have postoperative defecation problems. With pectopexy, there are no such problems. We have a longer lasting “follow up”.
The previous two patients have a very good postoperative course. This is a mutilating operation performed by skilled and experienced laparoscopists. In Macedonia it is only performed in our hospital.
The inserted mesh is permanent and non-resorbable. After we perform more such operations, we will publish our results in the international literature. We are working on a prospective, evidence-based medical study.
The successfully completed operation is our greatest pleasure”, says Dr. Hristijan Trpchevski, head of the endoscopic surgery department.

26 July 2022Laparoscopic surgery and hysteroscopic operations are more frequent at Hospital Plodnost. In 85% of cases, we have replaced classical surgery with laparohysteroscopy operations, which are more difficult for the doctors to perform, but provide greater benefits for patients. 10 June 2022New accomplishment of our operational team! Enormously large uterus with multiple fibroids was removed laparoscopically. The weight of the extracted material is 1000 grams.

Last year, we removed uterus weighing 1200 grams. Laparoscopists which remove uterus weighing over 1000 grams are called “Lions-laparoscopists”. Usually, these uteruses are operated in classic way with an operative incision. In our hospital, this is done laparoscopically.

8 June 2022Dermoid cysts are of congenital origin. They occur from the germ layers of the fetus and can develop on any body part. In our patient’s particular case, on both ovaries. They can grow later in life and manifest with pain and infertility. It is a pluripotent tissue and may contain bone, cartilage, pus, sebum, teeth, thyroid, etc. They can grow enormously, thus damaging the ovarian tissue.
The procedure is explained by Dr. Hristijan Trpcevski, who led the operation of the young girl:
“In this young girl, the right ovary had a cyst of 7 cm, and the left of 3 cm. The cysts were removed laparoscopically with preservation of the healthy ovarian tissue, so that the patient can be a mother in the future. 8 June 2022Laparoscopic removal of normal-sized uterus in under 30 minutes is already a routine procedure for us.
Our imperative are large uteruses reaching the navel, which we also remove laparoscopically, but the procedure is longer and lasts from 1.5 to 3 hours. In this particular case, the laparoscopic hysterectomy was performed in 25 minutes.

Stefan Matik and molecular biologist Simona Bardakoska, exchanged opinions with other gynecologists from Bulgaria and abroad, on their experiences in the field of human reproduction.

15 May 2022Paraovarian cyst (next to the ovary) was successfully removed in a young girl at Hospital Plodnost. This is a very typical case. The girl had laparoscopic puncture, i.e., draining the fluid from the cyst, elsewhere two months ago.
“The patient came to us in pain with a ultrasonography finding of 14-centimeter paraovarian cyst.”
“During exploration, the massive cyst was fused to the intestines and the omentum. Extensive adhesion and deliberation of the intestine is performed, after which the cyst is displayed. Two liters of fluid are extracted. A total cystectomy with ovarian conservation was performed, as the patient has not given birth yet.”
“This was a really difficult and extensive laparoscopic operation, due to the collision with the intestines, but in our hospital, this is already a routine. Previously, in similar procedures, larger cysts of up to 25 centimeters with 4-5 liters of fluid, have been removed.” said our laparoscopy expert, Dr. Hristijan Trpcevski after the procedure.

9 May 2022The Association of Nurses, Technicians, Midwives and Dental Nurses “Za nas” (“For Us”), decided to start celebrating this year’s International Nurses Week from Bitola and the host of this event was Hospital “Plodnost”, which is proven leader in IVF procedures, surgeries and great delivery conditions for 11 years. Also, this is the only hospital in the country that offers water birth delivery. The International Midwives’ Day, May 5, which is part of the Nursing Week, May 5-12, took place at Hotel “Premier”, with the attendance of midwives and nurses from across the country.
“We, nurses, are not angels, but we are certainly the second most beautiful thing after angels” is the slogan of the Association “For Us”. Therefore, we must fight for all midwives rights, who have the noblest profession in the world, to be the first in the act of bringing a new life to the world. However, the midwives, as well as the other nurses are on the sidelines. Only united together we can emphasize their importance and the place they deserve in the society “, said the president of the association, M.Sc. Gordanka Beshliovska.
She also praised the activities of Hospital “Plodnost” and its importance in all areas of gynecology, obstetrics, IVF procedures and surgery. As a host, Prim. At the event, the association “For us” gave special thank you plaques to Dr. Tase Trpcevski and the midwives of the Clinical Hospital – Bitola. Plaque was also handed over to Merlinda Drala, chief midwife of the “Plodnost” Hospital’s OB/GYN department, for her commitment in obstetrics advancement and unselfish sharing of her extended experience with all midwife associates in the hospital.
The celebration of May 5, International midwives’ day, was enriched with the performance of students from the State Music School in Bitola, Natalija Nikolovska, on violin, and Aleksej Belevski, on piano. 8 May 2022Another great success the Hospital Plodnost’s team is especially proud of.
Successful delivery by a mother with laparoscopically applied abdominal cerclage in the 12th week of pregnancy.
Due to congenital weakness of the uterus and despite the vaginal cerclages, the patient’s previous two pregnancies ended with miscarriage, at 20-25 gestation week of the fetus.
The only way for the patient to keep the fetus was to laparoscopically apply abdominal cerclage – which was done for the first time in North Macedonia by Dr. Hristijan Trpcevski, with the assistance of Dr. Sotir Ropi.
Today we have a completely different story. In 34 gestation week, a vital male baby is born by caesarean section with a birth weight of 2310 grams.
In the past experiences of Hospital Plodnost’s medical team, there are several mothers which have successfully delivered with previous intra-abdominal laparotomy cerclages (classic surgery) during pregnancy. There are also many preventive laparoscopic cerclages in non-pregnant women with congenital weakness of the endocervix.
We are proud to have written history – for the first time in our country, our team has placed an intra-abdominal cerclage during pregnancy by laparoscopic route which resulted in successful delivery of a healthy newborn.

7 May 2022Sentinel limph node dissection with indocianin green – in North Macedonia only at Hospital Plodnost.

The laparoscopy surgical team of Hospital Plodnost led by Dr. Hristijan Trpcevski, performed a total removal of the uterus with sentinel marking of the lymph glands with indocyanine green, in a 73-year-old patient. Lymph glands in the lesser pelvis were also removed.

“The main problem in this case is that the patient is short, weighing 130 kg. Excess weight makes such patients more difficult to be operated laparoscopically. We successfully performed this procedure laparoscopically, which is a great benefit for the patient despite the huge body mass. With classic surgery, recovery would have been very long. The patient left the hospital on the third postoperative day. Although the laparoscopic approach in this case was more difficult for us, for the patient it is certainly much easier. Our goal is to relieve patients. We used the advantage of laparoscopic marking of the lymph glands with indocyanine green contrast, a method which in North Macedonia is applied only in Hospital Plodnost.” said Dr. Hristijan Trpcevski.

7 May 2022Bozidar was conceived in our hospital by in vitro fertilization. The patient is 49 year old and has been struggling with infertility for a long time. She had several unsuccessful previous in vitro fertilizations elsewhere. This was her first and successful IVF in Hospital Plodnost.

7 May 2022The size of the myomas in the uterine cavity is a deciding factor for complete hysteroscopic removal in one step without reoperation. Myoma over 3 cm in a patient represents a risk for multiple procedures.
In an infertile female patient, a 5 cm myoma is removed in one act (with a single procedure). It is a large fibroid given the localization, which filled the entire cavity of the uterus and therefore have caused infertility. The risk of hysteroscopic removal is high, given the size of the fibroid and the inability to freely manipulate the resectoscope, as it may cause more bleeding.

The medical team, led by Dr. Hristijan Trpcevski, successfully removed the entire fibroid in one act without any complications. Otherwise, fibroids smaller than 3 cm in the uterine cavity are already a routine procedure for us. This fibroids size of 5 cm is a “cut off” for hysteroscopic removal. 7 May 2022The enormous desire to have a child was crucial for our patient, who despite her age, with her perseverance, faith and hope, gave birth to a baby boy. Given her age, this is a brave move of the proud mother, but also a great success for the IVF team of Hospital Plodnost. Over 10 previous unsuccessful in vitro procedures did not discourage the mother from intending to have a child.
“The crucial moment for our patient’s pregnancy was the endometrial receptivity test – the ERA test, to determine the receptivity of the endometrium. This way we had information about the exact time window for embryo implantation, which was the most important moment for our patient’s pregnancy. The success of this whole process really caused great joy to the patient, but also to our IVF team, which shows that persistence and knowledge provide top results.

This is not our first case of patients over 50 years of age becoming mothers by in vitro procedures. “So far, three patients at the age of 50, two at the age of 52 and one at the age of 51, have become pregnant through IVF procedures in our hospital and given birth to a child.” said Prim. D-r. Tase Trpcevski, after the great success they achieved together with the IVF team at Hospital Plodnost.

16 March 2022The laparoscopic team of Hospital Plodnost led by D-r. Hristijan Trpcevski, with another major success.

For the first time in Macedonia, the sentinel lymph nodes (the first glands closest to cancer) were marked and removed in an 85-year-old woman with endometrial cancer by laparoscopic procedure, which removed the uterus, fallopian tubes, and ovaries.

We first performed preoperative lymph node mapping with ISG 99 / isocyanide grin / and laparoscopic detection using highly sophisticated equipment (gamma camera and intraoperative 3D navigation). In that way we improved the visualization of the sentinel lymph nodes during the operation, enabling more precise localization, ie more accurate anatomical localization. This way, we remove all the somatic lymph nodes affected by cancer, both in the small pelvis (pelvic lymph nodes) and outside it (paraaortalni and paracavalni). In this way we determine the surgical stage of the disease, avoiding morbidity from complete lymphadenectomy.

The patient, although 85 years old, endured the operation well, which lasted 2.5 hours. He left the hospital on the third postoperative day. On the day of the operation, she was mobilized because she had only 3 small cuts of 5 mm on her abdomen. This is the latest trend in modern laparoscopic technology.
