Complex case resolved with advanced laparoscopy
A 30-year-old patient who had previously undergone surgery at another hospital during childbirth, where due to heavy bleeding she underwent a subtotal hysterectomy (the body of the uterus was removed, but the cervix was left).
Postpartum (after childbirth) the patient came to our hospital due to severe pain, where a large retroperitoneal hematoma was diagnosed, which at that time we treated conservatively – without surgical intervention.
However, despite the initial treatment, the pain did not disappear.
With detailed diagnostics, we determined that it was a 5 cm inflammatory tumor, which was located in the retroperitoneum and was in collision (contact) with large blood vessels. This was a consequence of the hematoma initially progressing and becoming infected, becoming an abscess.
Conclusion: With advanced laparoscopic surgery, through only two minimal 5 mm incisions we removed the encapsulated inflammatory abscess (a collection of fluid from the infection surrounded by a membrane/capsule).
This case represents a significant clinical case in minimally invasive surgery, due to the complex localization of the abscess and the risk of its close presence to blood vessels.
The patient left the hospital the next day without pain.
