Twins are two fetuses that develop simultaneously in the uterus. The incidence of twin pregnancies has increased in recent years due to the widespread use of assisted reproduction techniques.

A twin pregnancy is considered high-risk, as it carries specific and potentially serious complications that are not observed in singleton pregnancies. The risk of complications is directly linked to two key factors:

  • Chorionicity – The number of placentas
  • Amnionicity – The number of amniotic sacs

A monochorionic twin pregnancy (with one shared placenta) has a higher risk of complications compared to a dichorionic twin pregnancy (with two separate placentas). Therefore, ultrasound assessment of the number of placentas is a crucial factor in predicting perinatal outcomes.
 
Ultrasound Assessment of Chorionicity and Amnionicity
 

  • This evaluation should be performed before the 11th week of gestation and no later than 11-13+6 weeks, during the first-trimester fetal anatomy survey.
  • The long-term prognosis, likelihood of complications, and required follow-up differ significantly between monochorionic and dichorionic twins.

The fetal anatomy scan in twins can be performed in the first, second, and third trimester. The principles of examination are similar to those in singleton pregnancies, but twins require more frequent follow-up scans to assess:

  • Fetal growth
  • Amniotic fluid levels
  • Blood flow
  • Cervical length

The frequency of consultations is determined by early and accurate ultrasound assessment of chorionicity and amnionicity.
 
Advanced Ultrasound Imaging

  • The primary diagnostic tool is 2D ultrasound.
  • Under specific conditions, 3D images or 4D video recordings may be obtained during the scan. However, these images are not the primary focus of the examination.
